    Objective 

    To compare costs of an intensive care unit's (ICU) activity through two medico-economic classifications : the first leads to the actual hospital's budget negotiation ; the second is experimental.

    Design 

    Selection and description of intensive care unit stays.

    Classification with the Diagnosis Related Groups system, description and cost evaluation.

    Classification with a new system developed by the Société de Réanimation de Langue Française, the Société Française d'Anesthésie et Réanimation and the group Image of the Ecole Nationale de la Santé Publique, description and cost evaluation.

    Comparison.

    Setting 

    A 17 bed, medical/surgical ICU in a 875 bed urban teaching hospital.

    Patients 

    All patients admissions to an adult ICU over 1995.

    Interventions 

    None.

    Measurement and main results 

    The comparison study concerned 806 hospital stays.

    The hospital received 2,834,932 points with Diagnosis Related Groups and 4,108,108 with the new specific classification.

    The reason for the difference lies in a very poor accounting of patient severity and specific resource utilization in the Diagnosis Related Groups system.

    Conclusions 

    When their case-mix justifies a great number of intensive care beds, hospitals are really penalized by the actual budget system.
